Warfarin is chosen as therapy with monitoring strict International Normalized Ratio (INR) for reach effect optimal anticoagulation. During therapy is required evaluation periodically to factors that influence INR, including interaction medicine, diet, and compliance patient . Results : Regular INR monitoring with a therapeutic target of 2.0 –3.0 capable lower risk thromboembolism at a time minimize incident bleeding.
