RENAL AND HEART FAILURE INTERRELATIONSHIP IN HYPERTENSION
Abstract
Background behind : Hypertension is factor risk main the occurrence fail heart and disease kidney chronic . Both of these organs own connection closely related physiologically , where damage to one of them can trigger dysfunction in other organs through activation neurohormonal system , inflammation , and disorders hemodynamics . Relationship two direction This known as syndrome cardiorenal syndrome (CRS), which worsens journey clinical patients and improve risk morbidity as well as mortality . Case : A patient man 64 years old with history hypertension comes late with complaint progressive shortness of breath , swelling in the legs , and decreased amount urine . Examination show signs fail heart decompensation and decline function kidney acute . Echocardiography show faction ejection ventricle left decreased and there is hypertrophy ventricle left . Inspection laboratory show improvement serum creatinine and decreased rate glomerular filtration rate (eGFR), supports the diagnosis of CRS type 1. Results : Patient get therapy pharmacological in the form of diuretics , ACE inhibitors doses low , and arrangement fluid and pressure blood in a way tight . In five days care , occurs repair clinical with decrease symptom congestion , increase output urine , as well as decline level creatinine in a way gradually . Conclusion: In conclusion , the relationship between fail kidney and failure heart in context hypertension show importance detection early and management integrated . Approach multidisciplinary with focus on control pressure blood , optimization of volume status, and protection organ function is very crucial in repair external clinical patient with syndrome cardiorenal
